Chronic Care Management is a service covered by Medicare and most insurances which provides dedicated staffing, resources, and a direct phone number for enrolled patients. CCM services are provided to improve the management of chronic conditions. Our CCM coordinators are tasked with providing comprehensive and speedy response to patients enrolled in this premium service. As a patient enrolled in the CCM, you will receive a dedicated phone line, prompt response to any questions / concerns, and careful coordination of care for chronic conditions.
